striping the logical volume  and then re-create the tempdb required
devices, alter tempdb on the new devices with the same sizes as
before.

The ignore_dup_key and ignore_dup_row options to index creation are
useful.  But it seems like the most common thing you'd want to do is
disallowed:
create unique clustered index idx
